The Heart House at Hamlin Place Cardiac Transitional Care Program
The Heart House at Hamlin Place
Hamlin Place truly stands far apart from other skilled nursing rehabilitation facilities, in that we are most fortunate to offer the services of two locally renowned Board Certified cardiologists.
Dr. Michael J. Anesta, MD FACC with over 30 years practicing at Delray Medical Center, Bethesda Heart Institute and his Intracoastal Cardiology Center, in conjunction with Dr. Gary S. Shifrin, MD FACC, who himself brings 25 years practicing with Cardiology Associates of Palm Beach, JFK Medical Center and serving as Assistant Clinical Professor on UM’s Miller School of Medicine at FAU, coordinate your transitional cardiac care with our traditional rehabilitation programs.
The goal and focus of their direct care and involvement with your treatment is designed to reduce unnecessary re-hospitalizations and improve your prognosis and quality of life. Rather than interrupt progress in your program with off-site follow up visits, our two skilled specialists provide ongoing cardiac care right here at The Heart House of Hamlin Place.
